- Rural Innovation Hub (SCHC)
The Wellness Exchange serves as a centralized resource for:
- Obesity and chronic disease education
- Type 2 diabetes awareness and prevention

- Support services addressing social determinants of health
- Prevention programs and healthy lifestyle resources
By bringing trusted community partners together, the Wellness Exchange helps individuals navigate connected health systems with confidence and clarity.
Why This Matters
Obesity is a chronic disease that affects millions of Americans.
According to the American Diabetes Association:
- Nationwide, more than 2 in 5 adults are living with obesity.
- In Delaware, nearly 35% of adults are living with obesity.
- Obesity is linked to more than 200 related medical conditions, including type 2 diabetes.
The Wellness Exchange was created to increase awareness, improve access to care, and simplify the path to education, treatment, and prevention services.
