Managing high blood pressure doesn’t have to be overwhelming. With the right tools, support, and education, you can take control of your health and reduce your risk for serious cardiovascular conditions.

High blood pressure (hypertension) is often called the “silent killer” because it often has no symptoms while increasing your risk for heart disease, stroke, and other serious health conditions. The YMCA of Delaware’s Blood Pressure Self-Monitoring Program (BPSMP) helps adults with high blood pressure develop healthy habits and gain the tools they need to better manage their blood pressure and improve their overall health.

No Cost to Eligible Participants

Insurance may cover the cost of the program. If it doesn’t, the YMCA will ensure eligible participants can take part at no out-of-pocket cost.

How the Blood Pressure Self-Monitoring Program Works

Over the course of four months, participants will monitor their blood pressure at home and have 15-minute check-ins with a YMCA coach twice a month for support, education, and goal setting.

Virtual and in-person options are available. 

The YMCA’s Blood Pressure Self-Monitoring Program helps participants:

  • Track blood pressure at home with a free blood pressure cuff
  • Learn how everyday habits impact blood pressure
  • Meet regularly with a trained Healthy Heart Ambassador
  • Build healthier nutrition and wellness routines
  • Stay motivated with personalized support and accountability

Participating in the YMCA’s Blood Pressure Self-Monitoring Program can help you:

Plus: Participants who successfully complete the BPSM program will receive a complimentary one-month YMCA membership. This benefit is awarded upon fulfillment of all program requirements and is intended to support continued health, wellness, and active lifestyle habits beyond the program.


Who Can Join?

You may be eligible for the YMCA Blood Pressure Self-Monitoring Program if you:

  • Are 18 years or older
  • Have been diagnosed with high blood pressure and/or take blood pressure medication
  • Have not experienced a cardiac event in the last 12 months
  • Do not currently have atrial fibrillation or another arrhythmia
  • Do not have lymphedema or be at risk for lymphedema

The Blood Pressure Self-Monitoring Program is an evidence-based YMCA program that helps adults with high blood pressure learn how to monitor and manage their blood pressure through home monitoring, healthy lifestyle changes, and nutrition education.

How long is the program?

The program lasts four months and is designed to help participants build sustainable habits that support long-term blood pressure management.

How does the Blood Pressure Self-Monitoring program work?

Over the course of four months, participants will regularly monitor their blood pressure at home and meet with a YMCA Healthy Heart Ambassador twice each month for personalized support. During these 15-minute check-ins, participants receive education, review their progress, discuss challenges, and set goals to help improve their blood pressure and overall heart health. Both virtual and in-person participation options are available.

Is there a cost to participate?

No. There is no out-of-pocket cost for eligible participants. The program may be covered by your insurance plan. If insurance does not cover the cost, the YMCA will cover it so you can participate at no charge.

Who is eligible to participate?

Adults age 18 and older who have been diagnosed with high blood pressure and/or take blood pressure medication may be eligible. Additional health requirements apply.

What are the benefits of monitoring my blood pressure at home?

Home blood pressure monitoring can help you better understand how your daily habits affect your blood pressure, track progress over time, and provide valuable information to share with your healthcare provider.

Do I need my own blood pressure monitor?

Yes. Home blood pressure monitoring is a key part of the program. Participants will use a blood pressure monitor to track and record their readings throughout the four-month program. If you do not already have a monitor, the YMCA will provide one at no additional cost.

Can this program help lower my blood pressure?

Many participants experience improvements in blood pressure levels by consistently monitoring their readings, making healthy lifestyle changes, and applying the strategies learned throughout the program.

Is this program a replacement for medical care?

No. The Blood Pressure Self-Monitoring Program is designed to complement the care you receive from your healthcare provider. Participants should continue to follow their physician’s recommendations and treatment plan.
