Blood Pressure Self-Monitoring Program

Take Control of Your Blood Pressure with Support from the YMCA of Delaware

High blood pressure—also known as hypertension—is often called the “silent killer” because it often has no symptoms. Left unmanaged, it can increase your risk for heart attack, stroke, and other serious health issues. According to the American Heart Association, nearly 80 million adults in the U.S. have high blood pressure, and less than half have it under control.

The YMCA of Delaware’s Blood Pressure Self-Monitoring Program can help! This four-month, evidence-based program is designed to help adults lower and manage their blood pressure through simple, sustainable lifestyle changes. With tools, guidance, and ongoing support, you can take charge of your heart health from home.

How the Blood Pressure Self-Monitoring Program Works

Over the course of four months, participants will monitor their blood pressure at home and connect with a YMCA coach twice a month for support, education, and goal setting.

Virtual and in-person options are available. 

The YMCA’s Blood Pressure Self-Monitoring Program helps participants:

  • Track blood pressure at home with a free blood pressure cuff
  • Learn how everyday habits impact blood pressure
  • Meet regularly with a trained Healthy Heart Ambassador
  • Build healthier nutrition and wellness routines
  • Stay motivated with personalized support and accountability

Participating in the YMCA’s Blood Pressure Self-Monitoring Program can help you:

  • Lower your blood pressure
  • Reduce your risk of heart attack and stroke
  • Better understand the factors that affect your blood pressure
  • Build healthier eating habits
  • Feel more confident managing your health

Plus: Participants who successfully complete the BPSM program will receive a complimentary one-month YMCA membership. This benefit is awarded upon fulfillment of all program requirements and is intended to support continued health, wellness, and active lifestyle habits beyond the program.


Who Can Join?

You may be eligible for the YMCA Blood Pressure Self-Monitoring Program if you:

  • Are 18 years or older
  • Have been diagnosed with high blood pressure and/or take blood pressure medication
  • Have not experienced a cardiac event in the last 12 months
  • Do not currently have atrial fibrillation or another arrhythmia
  • Do not have lymphedema or be at risk for lymphedema
